Reception Information Sheet

All of us here at Upbeats DJ's, want to make your wedding reception a complete success. You can help us by completing this form.  With this information, your Upbeats DJ will be able to conduct your custom-tailored reception in a smooth and organized manner. We would like to have this form filled and email to us (by clicking submit when finished), no later that 30 days before your scheduled reception. This will help us build you custom itinerary and produce and well planed reception.

If you could also email us a digital photo of the both of you together. When we have the photo we place it on the cover of the itinerary to make it something for you memory book. email photo to upbeats@upbeats-dj.com

Once we receive this form we will then put together the event itinerary from the information gathered from this form and the consultation meetings.
